BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/ bcr-2013-200050 DESCRIPTION A 66-year-old woman with a sudden onset of right lower quadrant and right groin pain was admitted to our emergency department. Physical examination and laboratory tests were suspicious for acute abdomen. Ultrasonographic examination revealed a femoral hernia sac medial to the femoral vascular structures, fat tissue oedema and a dense cystic structure within the sac. An intravenous contrast-enhanced abdominal CT was performed for further evaluation. The whole appendix was depicted within the normal limits except for the distal segment (figure 1). The distal segment was located in the femoral hernia sac showing cystic dilation surrounded by a small amount of free fluid (figure 2). Those findings were consistent with acute perforated appendicitis in femoral hernia sac. The diagnosis was also confirmed surgically. De Garengeot’s hernia is a rare entity that is defined as the presence of an appendix in the femoral hernia sac. Different degrees of intestinal rotation or large caecum may lead to a pelvic appendix, which has a higher risk of entering a hernia sac of the pelvic peritoneum. The diagnosis of appendicitis in a femoral hernia was used to be extremely challenging and was almost never made preoperatively before CT. CT examination is an important radiological method that can clearly reveal appropriate diagnosis. In conclusion, perforated appendicitis in a femoral hernia remains a challenging diagnosis for the radiologists due to its extremely rare occurrence. The diagnosis should be considered in elderly women presenting with signs of a strangulated hernia. Appendiceal pus in a hernia sac simulating strangulated femoral hernia: a case report

Acute appendicitis involving the hernia sac is infrequent but well-documented in medical literature. In most instances, it occurs within the right inguinal (Amyand's hernia) or right femoral hernia (de Garengeot hernia). The diagnosis is always mistaken for incarcerated groin hernia. During surgery, the appendix itself, either perforated or strangulated, is most commonly encountered within the ...

Appendicitis in De Garengeot's Hernia Presenting as a Nontender Inguinal Mass: Case Report and Review of the Literature

Introduction. De Garengeot first described a femoral hernia containing the appendix in 1731. Appendicitis occurring in this rare clinical setting represents a challenge in diagnosis and management. Case Presentation. We present the case of a 71-year-old male with a three-day history of a nontender inguinal mass. Computed tomography (CT) suggested a femoral hernia. Intraoperatively, the hernia s...

Femoral Hernia with a Twist

In England, there were 4500 cases of femoral hernia for the year 2005-2006, accounting for 3.5% of all herniae [1]. The femoral hernial sac contents may be varied: preperitoneal fat, omentum, small bowel, or colon. In rare instances, the appendix can be found in the hernial sac, a reported incidence of 0.8%. Whether inflamed or not, the finding of vermiform appendix inside a hernia sac is known...

Strangulated femoral hernia containing a perforated appendix.

' 2004 Canadian Medical Association A 27 000 cases of femoral hernia occur yearly in the USA, accounting for 3% of all hernias. The hernial sac may contain preperitoneal fat, omentum, small bowel or colon and, rarely, the appendix, reported to occur in 0.8% of femoral hernias. To date, only 12 cases of perforated appendicitis in a femoral hernia have been documented, none in the past 13 years. ...
